1. Have you ever had a hard drive fail on you?

Yes, I've had plenty of personal drives and plenty of business related drive failures. Over 50 Deathstars for example.

So far the most reliable recent drive has been the Seagate 7200 series with 2 failures, and the failure wasn't catastrophic, just bad sectors and smart errors. The worst has been WD's special editions. I bought a lot since they were fast and only 75% are still running after 2-3 years.

2. Have you ever had a USB Pen fail on you? (if you've used these)

Yes, one the contacts finally wore to the point of not connecting anymore even after cleaning. Another after it was washed in the laundry.

3. Have you ever had an external Hard Drive Fail on you? (if you've used these)

Yes, a handful of maxtor externals died, and many more power supplies for them died. So far the seagate and WD externals have been fine.

4. Have you ever had a CD-R, DVD-R, or other optical media fail/corrupt on you?

Yes, scratches, labels peeling off and taking data with them, easy to split a DVD since it's 2 pieces. Also automated optical backup software sucks and will fail to do its job.

5. Have you ever deleted a file by accident and had it be unrecoverable?

No, due to proper backup procedures, shadow copy, and ontrack disk recovery tools

6. Do you have any sort of backup regimen?

External hard drives for media files, tape backup for financial and important documents. DVD archiving of photos that are also on external hdds. DVD archives I store in another location. RAID-1,5,10 on anything important and my personal workstations.
