I've played through it about 4 times. Great game. With the Enhanced edition I got 95hrs out of it.

I've seen weird issues with cheap HDMI cables and DVI to HDMI converters, but they were usually rolling horizontal lines and snowy static, not the squares that you are describing. But cables "on their way out" can do some weird things. Couldn't hurt to switch them out if you have more.
